Gear starts at a minimum of Item Level 148+ and Conduits start at Item Level 145. While adventuring in Zerith Mortis, you can also loot item level 226 green BoE items, to buy and sell on the auction house. The Sandworn gear also comes with unique broker-themed appearances and is item level 246. You can get Sandworn Relic from killing Zerith Mortis rares, and looting the Sandworn Chest once per day. Sandworn gear can be purchased from Rafiq, using Sandworn Relic.
Players engaging in the new zone of Zerith Mortis will find a few new types of gear. While the second one Crafter's Mark of the First Ones, instead allows for the crafting of a powerful ilvl 262 piece, but you can only equip one of that type. The first, Crafter's Mark IV, works similarly to Crafter's Mark II and allow the crafting of ilvl 233 gear.
